You will need to supply the agenda (which includes: course topic subject matter, time allotted for each subject, speaker names(s), and qualifications/biography).

Pesticide Recertification Course Attendance Reporting Sheet - The Pennsylvania Department of Agriculture generally emails this form to the person submitting the request for the recertification credits. These forms are then printed out for applicators to sign at the meeting to receive their credits.
