Armstrong
Program provides home visits to individuals w/developmental disabilities that occurred before the age of 22 yrs. Services can continue throughout a person's lifetime if needed. A nurse visits the home to do an intake and initial assessment to determine the needs and appropriate referrals for services. Assistance with follow-up and continued home visits are made on an as needed basis.
