Volunteer Screening Requirements
All volunteers working with minor youth must complete the following.
Option 1
The individual completes requested forms and returns them to the appropriate office. There are three forms--Information and Consent Concerning Consumer and Investigative Consumer Reports, Disclosure Statement, and Information Sheet. Penn State will pay all fees under option 1.
Option 2
The individual obtains a Pennsylvania criminal history check, a Pennsylvania child abuse clearance, and an FBI criminal history report. A copy of these reports must be submitted to the appropriate office. In addition, the individual must self-disclose any arrests or convictions on the Penn State Arrest and Conviction self-disclosure form. You will be responsible for any fees required. Copies must be within a six month period of time from the September 14, 2012.
If choosing Option 1, all forms must be returned to your county extension office by September 14, 2012. If you choose Option 2, copies of the three clearance results must be submitted to the Extension Office by September 14, 2012.
If you do not meet this deadline, you will not be able to continue volunteering with the 4-H program after September 30, 2012.
- In addition to the above screening requirements, all leaders must complete an annual mandated reporter training.
