We have new registration procedures this year. We suggest marking your choices in the check box beside the workshops on the reverse side as an aid in the registration process and as a reminder of your choices. Credit for MD applicators is being sought.
The sessions are designed to update you with the latest information concerning your role and responsibility in relation to pesticide regulations as you are earning core and category credits. The program is adaptable to any individual working with pesticides.
The registration fee of $75.00 covers instructional materials, lunch, breaks, and speaker expenses. Note that there are two times you can eat lunch. Only one copy of this form was mailed per business. Make photocopies for additional registrants. You may pay with a credit card
Please note: This school runs at capacity. So we must have your registration in advance.
Registrations postmarked after Monday, November 28, 2011, will be $95.00 per person.
