4-H Participation Form

The purpose of the Summary of Participation form is to enable members to keep a record of individual 4-H activities on an annual basis.
It introduces members to basic record keeping and self evaluation skills that are necessary throughout life. By completing this form, members will be better prepared
to complete resumes, scholarship applications, educational applications, etc.
If you want to submit this form to the Extension Office, you will be considered
for the Outstanding 4-H Girl and Boy Awards of the year. The form will be returned at the Achievement Event where winners will be announced and awards given. Everyone who submits the form becomes a member of the Green and White Club and will receive small tokens several times during the following year.
